Welders working outside their qualified range because the qualified man is on another bay. Preheat applied with a torch and judged by eye rather than measured with a contact pyrometer at the specified distance from the joint. Consumables left out of the oven overnight and returned to the rack in the morning. Grinding used to blend a weld toe into acceptability rather than to prepare a repair.
On structural steelwork the recurring finding is welder continuity. Vietnamese fabrication shops carry real turnover, and it spikes after Tet, when a proportion of the workforce does not return and new labour arrives in February. A welder qualification record dated December means nothing if that welder is no longer on site, and continuity periods lapse quietly. The weld map with a named welder per joint is the control, and it only works if somebody checks the names against the current qualification register.
On skids and packaged equipment the recurring finding is interface control. The vessel is right, the piping is right, the structure is right, and the three do not meet. Verifying interfaces on the shop floor against the model, and confirming them by measurement before break-down for shipping, is a great deal cheaper than discovering it at site. Where the interfaces are complex or the module is going into an existing plant, 3D laser scanning and as-built capture at the trial assembly stage removes the argument entirely.
Material traceability and the EN 10204 3.2 trap
Vietnamese fabricators buy plate, sections and pipe from Chinese, Korean, Japanese and domestic mills, and the paperwork quality varies accordingly. The verification is mechanical: match the heat number stamped or stencilled on the material to the certificate, confirm the certificate is the right type, and confirm that when the plate is cut the marking is transferred to every piece before the original mark is lost. That last step is where traceability is broken most often, and once it is broken it cannot be restored.
The commercial trap is the certificate type. EN 10204 type 3.1 is issued by the manufacturer's own independent inspection department. Type 3.2 requires validation by the purchaser's representative or an independent inspection body, in addition to the manufacturer. Buyers routinely write 3.2 into the purchase order because it appears in the parent specification, then appoint nobody to countersign, and 3.1 certificates arrive instead. Expediting: reading the delivery date from the shop floor
A progress report shows what the shop wants you to see. A weekly presence in the fabrication bay shows what is actually happening, and the two diverge earliest at the point where recovery is still possible. The leading indicators are physical: plate that has not arrived, a bay standing idle, a heat treatment queue at a subcontractor, a rising weld repair rate, blast and paint work stalled because the humidity has been above the coating window for a week.
That last one is specific to the region and it is chronically underestimated. Through the wet season, ambient humidity around Ho Chi Minh City routinely pushes the steel temperature to within three degrees of dew point, which closes the coating window. It is to require that each waiver is a written decision by a named person with the residual risk stated, and to convert the waived point into a documented review at the next available stage.
