CP-189 is an American National Standard and it states requirements where SNT-TC-1A offers recommendations — it was written because employers implemented SNT-TC-1A inconsistently. It sets minimum education, training, experience and examination requirements for Level I, II and III nondestructive testing and predictive maintenance personnel, and adds requirements for NDT instructors. A validation that finds the report sound is as useful as one that does not: the point is a defensible answer, not a defect.

What does a LT report have to show under CP-189?

Missing any one of those makes the examination unprovable rather than merely undocumented.
What most often makes a LT report indefensible?

It is a records failure rather than a technique failure — the examination was very likely performed correctly, and the evidence supporting it cannot be produced, which under CP-189 is treated the same way as not having done the work.
Can a LT report be validated without re-inspecting?
Partly. A document review establishes whether the examination as recorded was adequate and correctly interpreted, and whether the personnel and equipment evidence holds. It cannot establish that the examination was performed as recorded, because LT leaves no reviewable data set — for that, targeted re-inspection is the only answer.
Who is qualified to validate a LT report under CP-189?
A Level III qualified in LT specifically. Method scope is the constraint people miss: a Level III qualified in another method cannot approve or credibly challenge a LT interpretation, and an opposing reviewer will test that first because it is documentary and binary.
How far back do CP-189 records have to reach?
CP-189 makes recertification a requirement rather than a recommendation, with the interval fixed in the firm's certification procedure. The dependent credential has a firm published number: the ASNT NDT Level III certificate the CP-189 Level III must hold is renewed every five years, either by accumulating a minimum of 25 recertification points across the 60-month cycle while maintaining at least 36 months (756 days) per method of active Level III work, or by retaking and passing the examinations before the expiry date. ASNT publishes renewal-by-points fees from US$440 (member, single method) and examination recertification fees of US$525–545 per method for members. Where a validation is being done for a dispute or a purchase, the retention period is what decides whether the question can be answered at all — records that were never kept cannot be reconstructed afterwards, and a reconstruction is worth markedly less than a contemporaneous record.
What happens if the validation finds the report was correct?
That is a normal and useful outcome. The buyer gets a defensible position to accept the work, pay the invoice, close the claim or proceed with the purchase — supported by an independent review rather than by assertion. A validation is commissioned to settle the question, not to find a defect.

Employer-based, with a central-certification prerequisite at the top. Levels I and II are certified by the employer against the firm's written certification procedure. The Level III running that programme must additionally hold ASNT NDT Level III certification in the method — a credential from ASNT Certification Services, earned through a computer-based Basic examination plus at least one Method examination, and ANAB-accredited to ISO/IEC 17024.
